  By mail

Simply return your cheque or credit card payment along with the tear-off payment slip at the bottom of the renewal notice we send you, in the reply paid envelope provided.

Pay in person at Australia Post

Making changes to your policy

For your convenience we have included below some of our most commonly used forms to enable you to request changes to your policy.

 Changing my address/name

Changing your name or address can only be done at the request of the policy owner(s). Simply download the Change of Name/Address Form, complete and return it to us at our Reply Paid address.

Apply to increase my sum insured/add new benefit

Applying to increase your sum insured, (or add a new benefit to a Term Life Insurance or Family Protection Policy) can be done by downloading the relevant form below, and completing and returning it to us at our Reply Paid address.

Term Life Insurance

Family Protection Policy

Applications for increases or to add a new benefit are subject to our normal underwriting guidelines and must be within the applicable premium/sum insured limits for that product type. After receipt of the application for an increase in cover, in come cases no increase will be offered or may only be offered on special terms.

 Decrease my sum insured

You are able to decrease your sum insured. Please be aware that different policies have different sum insured and premium limits, so first check with us what your options are. You can do this by calling us on 132 623. When you know what sum insured you require, the policy owner can request this change by downloading the Request for Sum Insured Decrease form here and returning it to our Reply Paid address.

 Changing to monthly premium payments

Paying your premiums by the month can make it easier to manage your budget. You can have premiums deducted each month from your credit card or nominated bank account. If you wish to change from an annual to monthly premium payment, download the Direct Debit Request Form, complete and return it to us by faxing it to (02) 9222 1764 or by sending it to our Reply Paid address.

Remember that a change from annual payment of premiums to monthly will mean that you will no longer have the discount associated with paying premiums annually.

Change in policy ownership or beneficiaries

Changing the ownership or beneficiaries of the policy can only be done by the current policy owner(s). To transfer ownership of a policy, simply complete the Memorandum of Transfer form which appears on the back of the Policy Certificate, then return this to us at our Reply Paid address. If you cannot locate the Policy Certificate, please call us on 132 623.

The policy owner(s) can change the beneficiaries on a Term Life policy by completing and returning the Nomination of beneficiary details (MBF Term Life insurance).
